This reminded me of a similar issue posted recently, and I see you posted there, so you are on the right track Forms and loops: Can I use the same survey at multiple times with a loop?
To unpick this more - is the form filled in even on first presentation, or is it that it is empty on the first trial but then the answers from the previous completion carry over?
