jsPsych version (e.g. 7.3.1): After creating the form questionnaire, try to run the program. If the program does not report errors, it cannot run (flash back).
Are you definitely using jsPsych, not PsychoJS?

Like this, the program exits as soon as it runs
Your photos show that you are running PsychoPy locally rather than jsPsych online.
I think you have the same blank row in form spreadsheet error as a number of your fellow students.
