It looks like there is a substantial problem with the refresh rate (bolded above)…

You should check your graphics card settings. Is it set to sync with the vertical refresh? Perhaps provide a screenshot of that control panel for clarity.

It turned out that the refresh rate of the monitor was set at 100 Hz. Taking it back to 60 Hz solved the problem. Thanks for your assistance!
